Eligibility
Who Qualifies for Connecticut's CFC Program?
CFC eligibility is based on three criteria: Medicaid enrollment, functional need, and level of care.
Requirements You Must Meet
Be enrolled in Connecticut Medicaid — HUSKY A, C, or D
Need assistance with at least 2 activities of daily living
Meet institutional level-of-care criteria (equivalent to nursing-facility care)
Live in the community — not in a nursing home or institution
Any age — no minimum or maximum age restriction applies

Cottage Homecare Services CT LLC is a contracted provider of Adult Family Living (AFL) and other Medicaid-funded programs, allowing Connecticut residents to hire their own caregiver anywhere in the state.

Our registered nurse conducts a home assessment and documents your care needs for DSS. We guide you through all required paperwork to ensure a smooth approval.

Care Begins
Once approved, your chosen caregiver is enrolled and trained. Care begins in your home on a schedule that works for you — paid through the Medicaid program.
